With several medical doctors on staff, the Alliance is deeply rooted in traditional Western medicine and utilizes the best practices and tools available to perform a comprehensive medical evaluation as part of its approach to health and healing.
While science and technology are both important components in our approach, we also understand that the practice of good medicine begins with good listening. The first thing our staff of caregivers will do when meeting with you is listen, and ask good questions. What are your top health concerns at the moment? What does your medical history look like? Your family history?What is going on in your life at the moment? How are your stress levels? By understanding you as a whole person, rather than simply trying to fix an “isolated” pain or ache, we can better partner with you and assist you in achieving optimal health and well-being.
What We Need for Your Initial Visit
Your Alliance Institute Medical History Form. Please take the time to carefully fill out your Alliance Institute Health History Form and bring it with you to your first appointment. This will give your healthcare provider valuable background information.
- Pertinent past medical records and diagnostic test results. Please have your relevant records sent to us or bring copies to your first appointment. These results will enable you to get much more out of your first visit, as your doctor will be able to assess your circumstances more accurately and prescribe treatment modalities more effective for you.
What to Bring
Please bring the following to your initial visit:
- All of your medications and supplements in the original bottles, or a typed list of these items to keep with your medical records.
- Your questions and concerns. These are of primary importance to us. If they are not adequately reflected on your completed questionnaire, it might be helpful to write a small list of primary concerns to bring with you.
- Please take time to carefully fill out the forms on the right side of this page (only complete Minor's Form if patient is under age 18).
- A support person (if you'd like).
What to Expect During Your Visit
Once you have arrived at the Alliance Institute for Integrative Medicine and checked in, we will:
- Introduce you to your care provider.
- Take time to review and understand all of your health concerns, and work together to prioritize those of most concern.
- Assess your overall condition.
- Begin to develop a personalized health plan for you, and identify any other members of our interdisciplinary team that we may wish to discuss your plan with (e.g., chiropractors, massage therapists, etc.).
- Most times you will be offered an introductory acupuncture treatment if you wish.
- Help you schedule a follow-up visit.
- Discuss next steps.
Consultative Services
Please understand that Integrative Medicine consultations do not replace the care of a primary care physician and/or appropriate specialist. To this end, the Alliance Institute for Integrative Medicine will collaborate with your physicians to integrate your existing care.
